Written service agreement dated 3 Jun 2026. Not registered for GST, so no GST is charged on this invoice.

Thank you for your business!

The names, NDIS number, ABN, bank details, rates and dates are made up for this page: the ABN and BSB fail their checks on purpose and the NDIS number is a plain run of digits. Use the layout, not the figures.

What must be on an NDIS invoice, and who says so

Three voices ask for things on an invoice: the NDIS, the ATO and the plan manager. Their lists are not the same, so this page keeps them apart.

How the plan is managedPlan-managedWhat the NDIS describesAn invoice to the plan manager. It must include a valid ABN unless you are exempt from providing one, and a Statement by a supplier form goes with it if you are. The claim the plan manager enters needs the participant name, the participant reference number or NDIS number, the dates of support, the support item reference number and the support item price.Where it goesDirectly to the participant's plan manager, who enters your claim in the relevant provider portal.
How the plan is managedSelf-managedWhat the NDIS describesAn invoice to the participant, then a receipt once they have paid. The NDIS pages we read give no list of what a provider's invoice must show. The participant's own records need, among other things, your name and ABN (or the reason you are exempt), the start and end dates, and a receipt or tax invoice.Where it goesTo the participant, who pays it through the participant portal.
How the plan is managedNDIA-managedWhat the NDIS describesA payment request in the myplace provider portal, rather than an invoice. The claim needs the same details as above.Where it goesMade in the portal by a provider who is registered and recorded as a "my provider".

What the NDIS pages do not say
Beyond the ABN and the details a claim is built from, the NDIS pages we read do not list what a provider's invoice must show. The 2025-26 pricing arrangements say plan managers must always ensure a valid tax invoice, with relevant information about the supports, is included for all payment requests. Plan managers fill the gap with their own lists, covered below.

The five details a claim is built from, and where each one sits on the sample invoice above:

Participant name
The person with the plan, in full. The sample prints the participant as the client.
NDIS number
The NDIS calls it the participant reference number or NDIS number. The sample prints it under the participant's name.
Dates of support
One line per support, each with its own date, shows them clearly. The sample has a date column.
Support item number
From the NDIS Support Catalogue, written as the catalogue writes it. In the 2026-27 catalogue (version 1.1), 01_011_0107_1_1 is "Assistance With Self-Care Activities - Standard - Weekday Daytime", and the Saturday version of the same support has its own number, 01_013_0107_1_1. Provider travel kilometres for those items use 01_799_0107_1_1. Check each number against the current catalogue.
Support item price
The price for one unit, such as an hour, with the quantity. For plan-managed and NDIA-managed supports the Pricing Schedule 2026-27 sets a maximum for each item and says each maximum already includes any GST. This page does not print the maximums, because they are reviewed and a copy goes stale.

When the ATO tax invoice rules apply

The ATO's tax invoice rules are for taxable sales. A supply to an NDIS participant can be GST-free when all of the ATO's conditions are met, and the ATO's GST rules apply to a business that is registered for GST or required to be.

For a taxable sale, the ATO says a tax invoice must give enough information to clearly determine these seven details:

It says it is a tax invoice
The document is intended to be a tax invoice.
The seller's identity
Your name or business name.
The seller's ABN
Your Australian business number.
The date it was issued
The day the invoice was issued.
What was sold
A brief description of the items, with the quantity if applicable and the price.
The GST payable
The GST amount, if any, payable.
How much is taxable
The extent to which each sale on the invoice is a taxable sale.

For larger sales the ATO also asks for the buyer's identity or ABN; its page gives the amount. A tax invoice that mixes taxable and non-taxable items must clearly show which items are taxable. If a customer asks for a tax invoice for a taxable sale, the ATO says you must provide one within 28 days, and it names a small-sale exception.

Whether your NDIS invoice is a tax invoice depends on whether you are registered for GST and whether any line is a taxable sale. Our guide to NDIS supports and GST goes through the ATO conditions, and Australian tax invoice requirements is our summary of the ATO checklist. This is general information, not tax advice: ask your adviser.

What plan managers usually ask for on top

These are not NDIS rules. They come from the invoice pages of five plan managers, read on 24 October 2026, and each plan manager differs. Ask yours for its list before the first invoice.

Asked for by most
A unique invoice number, the invoice date, your bank details, your contact details, the service dates, a description of each line with the hours or quantity and the rate, and the total.
The support item number on each line
Four of the five ask for it, one of them "if known". Copy it from the Support Catalogue as written.
A date on each line
Provider Choice asks for an individual date on each line rather than a range. Plan Partners asks for the date or spread of dates you delivered the service. One line per support with its own date suits both.
The participant's name, not a parent's
Plan Partners asks for the invoice in the name of the person with the plan, not a parent and not the plan manager.
The NDIS number as nine digits
Provider Choice asks for the number as nine digits with no spaces.
An ABN that matches ABN Lookup
Plan Partners says the ABN on the invoice must match ABN Lookup.
A PDF
Plan Partners asks for a PDF or JPEG with no links, Leap in! suggests saving the invoice as a PDF, and My Plan Manager prefers a PDF.
An invoice, not a quote or a statement
My Plan Manager says it can only process invoices.

Laying out the lines

One line per support, each with its own date
It shows the dates of support the claim needs, and it lets a plan manager match each line to a support item.
Travel and cancellations as their own lines
The 2025-26 pricing arrangements give an example where a claim in three parts "should be shown separately on their invoice to the participant and claimed for separately". The sample shows travel time and travel kilometres on their own lines. See travel and cancellations on NDIS invoices for the conditions.
GST-free and taxable lines on one invoice
If you are registered for GST, one invoice can mix both, and the ATO says the invoice must clearly show which items are taxable. The sample is from a support worker who is not registered, so no GST is charged.
The rates are examples
The rates on the sample are made up for the page. For plan-managed and NDIA-managed supports, read the maximum for each item in the Pricing Schedule 2026-27, not here.

Fill it in and download a PDF

The free generator has a field for each part of the sample: the participant and NDIS number, a "c/o" line for the plan manager, a dated line per support with its item number, hours and rate, and GST switched on or off. It makes an A4 PDF in your browser with no signup. It does not check item numbers or prices, and this page has no Word or Excel file.

Questions about the NDIS invoice template

What should an NDIS invoice include?
For a plan-managed participant: your ABN (or the Statement by a supplier form if you are exempt), the participant's name and NDIS number, the dates of support, the support item number and the price, with one line per support. Plan managers usually add a unique invoice number, the invoice date and your bank details. The sections above say which of these the NDIS asks for and which are a plan manager's own practice.
Is there an NDIS invoice template in Word, Excel or PDF?
This page has no Word or Excel file to download. It shows a sample invoice, and the free generator fills one in for you and downloads it as a PDF. Several plan managers also publish their own PDF or fillable templates on their provider pages.
Does an NDIS invoice have to be a tax invoice?
The ATO's tax invoice rules apply to taxable sales. A supply to an NDIS participant can be GST-free when all of the ATO's conditions are met, and the ATO's GST rules apply to a business that is registered for GST or required to be. The NDIS asks for a valid ABN on an invoice to a plan manager. If you are registered and a line is a taxable sale, the ATO's seven details apply to it. Ask your adviser which applies to you.
Can I copy the sample invoice as it is?
No. Its names, NDIS number, ABN, bank details and rates are made up. Use it for the layout, then check each support item number against the current Support Catalogue and each price against the Pricing Schedule 2026-27.
Can I put a date range on one line?
The NDIS says a claim needs the dates of support. Plan managers differ: Provider Choice asks for an individual date on each line rather than a range, and Plan Partners asks for the date or spread of dates you delivered the service. One line per support with its own date suits both. Ask your plan manager.
Who do I send an NDIS invoice to?
It depends how the plan is managed. A plan manager gets the invoice for a plan-managed participant, the participant gets it if they self-manage, and for NDIA-managed funding a provider who is registered makes a payment request in the myplace portal instead. Our guide to [invoicing NDIS participants](/invoicing-ndis-participants-au) goes through each.
Why was my invoice sent back?
It can be a plan manager checking its own list, or the NDIA system rejecting a payment request with an error code. [Why an NDIS invoice or payment request can be rejected](/ndis-invoice-rejected-au) groups the codes by cause with the fix for each.
